Martial Arts is Dangerous - I understand that training, practicing or participating in martial arts is dangerous and that I must at all times abide by the rules of Tempo JJ and any instruction provided to me by those instructing me and obtain clarification before proceeding if those rules or instructions are not understood.
I acknowledge that during all such times whilst in a Tempo JJ class, both my property and my person shall be at my own risk and I will not hold Tempo of its Instructors liable for any personal injury or loss of property, whether caused by negligence of Tempo, its servants or agents or otherwise.
I warrant that I am physically and medically sound to proceed with a normal course of exercise.
Authority for First Aid, Medical Treatment - In signing this form I AUTHORISE THE INSTRUCTOR/S AND OTHER OFFICIALS to give first aid to me in the event of injury and to arrange for me to be transported to Hospital for medical treatment. In an emergency, Tempo JJ reserves the right to call an ambulance and cannot be held accountable for costs involved nor for any other ramifications of taking such action.
I indemnify Tempo JJ in relation to liability for any damages suffered by me.
I give Tempo JJ the rights to use and publish photos and/or videos of myself.
